Advantage Transition Agency LLC work with long-term care facilities to transition individuals who health has improve for during their stay at the long-term care facilities. We coordinate services to ensure that members have a level of care that will promote their independence in the community.
Advantage Transitions Agency LLC is here to provide our member with all the information they need to help them make informed decisions about services to help them improve their physical, emotional, and financial independence when they opt into living in a community setting.
Advantage Transition Agency LLC advocates for the safe and smooth return of our members from a long-term care facility to their community. We also strive to remove the obstacles that delay our members to live an independent and comfortable life in their own residence. Therefore, we offer a variety of services to improve or maintain the quality of life of our members.
Our process begins with assessing the individual needs and choices of our members. After the assessment is completed, we will coordinate with our members in identifying the care options that will fully support their health requirements. Medicaid-funded program in Colorado designed to help
individuals receive care in the comfort of their own homes.
Personal Care and Homemaker Services - BI/CFC/CIH/CMHS/EBD
Designed to support individuals in performing daily activities they
are unable to do on their own due to age, disability, or medical condition.
Homemaker Agency/Extraordinary Cleaning CES/CFC/SLS
Intensive housekeeping service to address needs related to a
participant's disability or health condition.
Residential Habilitation Services-IRSS/Host Home DD
Offers 24/7 support to adults with intellectual and developmental disabilities.
Day Habilitation-Supported Comm Conn-DD/SLS
Designed to support adults with intellectual and developmental disabilities (IDD) in
acquiring, retaining, and improving self-help, socialization, and adaptive skills.
